Please download the new patient paperwork from the link below and complete and bring to the first visit. I do have copies at the office if you are unable to print/complete prior to your appointment. Please make sure you read Page 3

As lifestyle counseling is a primary focus of my treatment, I encourage patients to communicate to me their concerns and questions during active treatment. I offer my email as a cost-effective and time-saving option to maintain communication with patients. There is no charge for email questions, but it should be understood and agreed this communication cannot replace a consultation.

I do my best to answer emails in a timely fashion. If the email question requires lengthy discussion, clinical decision making or record keeping you will be notified that an office visit or telephone consultation must be scheduled. My office is located in my home in Niagara Falls ON, near Lundy's Lane and Dorchester Road. As this is my private residence it’s important that you arrive on time as I do not have a waiting area.

Parking is available in the driveway or on the street. There are stairs, if this presents a concern please reach out to make alternative arrangements.

Your time is valuable to us, please respect your appointment time. If you need to reschedule your appointment, kindly provide 48 hours notice.
